If you then decide to do so, please complete TASC’s Mental Health Support Application Form. Please note that when filling in your application form, you’ll be prompted to provide information from your self-assessment results.

To apply for support, please download a TASC application form below and provide as much detail as possible to help us understand your circumstances and recommend the most appropriate support option for you. You can also fill in the application form on behalf of someone else, if you have their permission to do so.
